If you have a question regarding a Blue Cross Blue Shield claim, you may contact the Blue Cross Blue Shield Member Service office at the phone number that is listed on the front of your ID card or call MIIA Health Benefits Trust at 1-800-374-4405.

Q. How do I file a claim?

A. Participating providers will file claims for you. You may also call Member Service at the number on your ID card to request a claim form. You may also call MIIA Health Benefits Trust at 1-800-374-4405. Send the completed form and an itemized receipt from your provider to the address listed on the claim form. Q. How do I find out if a Prescription is Covered?

A. Please visit bluecrossma.com and click on "Pharmacy Program" for detailed information on prescription drug benefits and related procedures.

Q. What if I receive a Subscriber Claim Summary in the mail?

A.When Blue Cross Blue Shield sends you a Subscriber Claim Summary, it is to explain how they processed a claim. It is not a bill, and you should not send any payment to MIIA or to Blue Cross Blue Shield. If there is a balance listed, the health care provider will notify you of your responsibility.

Q. What if I get a bill from my doctor?

A. Many times, when you receive a bill from your doctor, it is for your co-payment, co-insurance, or deductible. These are common features of health plans. For example, some health plans require that the member pay $10 for an office visit and the rest is covered.

If you have additional questions, the Blue Cross Blue Shield Member Service office can also help you. Please be sure to have your ID number, health care provider's name, and the date of service handy when you call. You may also call MIIA at 1-800-374-4405.

Q. What happens when I need a new ID card?

A. It is important to carry your ID card with you at all times. Your MIIA Blue Cross Blue Shield card is recognized around the world. If you lose your card and need a replacement, simply call Blue Cross Blue Shield Member Service. If you have a family plan, and have access to another family member's ID card, you can call the telephone number shown on the front of the card. If you do not have access to another family member's card, you should call 1-800-782-3675.

Q. What if I want to know if something is covered?

A. The first place to check is your benefit materials. In most cases, this is called either the Member Handbook, Subscriber Certificate, or the Summary of Benefits. Materials are organized in general categories such as inpatient care, outpatient care, surgery, medical equipment, prescription drugs, etc. Virtually all questions are waiting to be answered with a quick check of your benefit materials.

When you call, it is important for you to know the specific type of service involved so that a representative can help.
