How do I file a claim?

Participating providers will file claims for you. You may also call Member Service at the number on your ID card or MIIA Health Benefits Trust at 1-800-374-4405 to request a claim form. How do I find out if a Prescription is covered?

Go to your MyBlue app on your smartphone or go to bluecrossma.org and sign in to MyBlue to find out if a Prescription is covered.

What if I want to know if something is covered?

The first place to check is your Summary of Benefits. Materials are organized in general categories such as inpatient care, outpatient care, surgery, medical equipment, prescription drugs, etc. Virtually all questions are waiting to be answered with a quick check of your benefit materials. If you have remaining questions, you may contact the Blue Cross Blue Shield Member Service office at the number listed on your ID card or call MIIA Health Benefits Trust at 1-800-374-4405. When you call, it is important for you to know the specific type of service involved so that a representative can help.

What if I receive a Subscriber Claim Summary in the mail?

A Subscriber Claim Summary is sent to explain how Blue Cross Blue Shield processed a claim. It is not a bill, and you are not required to make a payment based on this document. You will receive a bill from your health care provider when a balance is due. For further information, go to bluecrossma.org and sign in to MyBlue or go to your MyBlue app on your smartphone. What happens when I need a new ID card?

It is important to carry your ID card with you at all times. If you lose your card and need a replacement, simply call Blue Cross Blue Member Service at 1-800-782-36758. You may also contact MIIA at 1-800-374-4405.

What if I get a bill from my doctor?

Many times, when you receive a bill from your doctor, it is for your co-payment, co-insurance, or deductible. These are common features of health plans. For example, some health plans require the member to pay a share of the cost for an office visit, and the rest may be covered. If you have additional questions, Blue Cross Blue Shield Member Service can also help you. Please be sure to have your ID number, health care provider’s name, and the date of service handy when you call.
