Blue Cross CEO discusses healthcare affordability challenge on MMA podcast

During a recent episode of the MMA's The 351 podcast, Blue Cross Blue Shield of Massachusetts (BCBSMA) President and CEO Sarah Iselin discussed the growing challenge of health care affordability and the factors driving the rising rates. For municipalities, the issue is a significant concern as health insurance costs continue to put pressure on local budgets and compete with other essential community investments.

"We have been experiencing the fastest growth in the cost of health care that we've seen in at least two decades," Iselin said. Multiple factors contribute to today's cost pressures, including rising pharmacy costs due to high-cost specialty medications and the expanded use of GLP-1 drugs. Also, hospitals and health systems are facing higher labor and operating costs while demand for inpatient and outpatient care has increased.

Iselin acknowledged that municipalities face unique challenges in responding to these increases. Unlike private-sector employers, cities and towns operate under tighter revenue constraints, making it more difficult to absorb sustained growth in health care costs.

"It's a real challenge for state and municipal budgets," Iselin said. "This growth in the cost of health care is making it really hard to make choices between investing in schools and libraries and public safety, and repairing roads and keeping bridges up to code."

Through partnership with BCBSMA, MMA and MIIA continue to work with municipal employers to evaluate plan options, explore innovative cost-management strategies, and promote employee health and well-being. Efforts include pharmacy cost-management initiatives, data-driven plan design strategies, virtual-first care options, investments in preventive care, and wellness programs that support long-term health outcomes.
